Our Teams
Assessment, Planning and Sourcing Support
Community Social Care Teams
The Community Social Care Teams in Cheshire West and Chester work in different community areas to support adults who do not need the support of the dedicated Learning Disability and Mental Health Teams. The teams undertake Care Assessments with adults, and their carers if needed, and help them to plan their own support, as well as any further assessments relating to legal processes if needed. The teams also undertake safeguarding enquires that are not connected to an organisation (in these instances, enquiries are managed by the Adult Safeguarding Team).
Home Assessment Team
The Home Assessment Team works with people who have recently left hospital to provide assessments and plan their long-term support with them. The team works with NHS and Voluntary and Community Sector services in our local Community Response Hubs to make sure that people receive the right type of support and rehabilitation at home if they need it.
Learning Disabilities Team
The Learning Disability Teams support adults whose greatest needs come from a Learning Disability. The teams undertake Care Act Assessments with adults with a learning disability, and their carers if needed, and help them to plan their own support, as well as any further assessments relating to legal processes if needed. The team work with people at home and in hospital and undertake safeguarding enquires that are not connected to an organisation (in this instances, enquiries are managed by the Adult Safeguarding Team).
Mental Health Teams
The Mental Health Teams work with adults who are experiencing mental health problems. They undertake Care Act assessments for people, and their carers if needed and work with people at home and in hospital to help them get the right support in place. They work closely with our Mental Health Provider Service to maximise independence and undertake safeguarding enquires that are not connected to an organisation (in this instances, enquiries are managed by the Adult Safeguarding Team).
Community Occupational Therapy Service
Occupational Therapy is delivered in a number of teams however we also have a dedicated community occupational therapy team who provide services to adults and children, who are living at home with complex, long term and permanent disabilities. The team helps people with a disability to live as independently as possible by giving advice on available equipment options, support for people with physical disabilities and their carer and, if appropriate, the provision of adaptations to their home. Visual Impairment Team
The Visual Impairment Team undertakes specialist assessments for visually impaired children and adults. They are responsible for providing rehabilitation, training and working with people to get the right specialist technology, equipment and adaptations for their home to promote their independence and ability to perform day to day activities. They also maintain the register for people who have a visual impairment.
Integrated Care Connections Team
Our Care Connections service source and secure care solutions for people, sometimes referred to as ‘Brokerage’. They work with providers of care at home and care homes across Cheshire West and Chester (and beyond when needed) to find the right type of support to meet a person’s needs. The team works across health and social care to support people at home with, or without, a learning disability or mental health diagnosis as well as supporting people who need care to leave hospital.
