Children and young people’s community healthy weight pathway
The purpose of this pathway is to ensure that children and young people with a BMI outside of a healthy range are supported using a consistent approach. This pathway is for use by all professionals working with children and young people between the ages of two and 19 (extending to 25 years for young people with special educational needs or disabilities). For children from birth to 23 months there is a separate pathway for healthy weight.
The pathway has been developed by a multi-disciplinary team of partners across the NHS, local authority and CVS sector. The document below outlines the pathways for children who are weight and measured as below a healthy weight, including how to identify a child’s weight, safeguarding procedures and support services via the appropriate pathway.
Please note the tier two pathway (page 9) is a model pathway there are currently no community weight management programmes for children and young people in the borough.
