What is a Carer's assessment

If you provide regular unpaid support to someone, whether that be a friend, a family member or loved one who could not manage without it, you have the right to have a Carer’s assessment under Care Act 2014.

This assessment gives you the chance to tell us about what it is like to be Carer, what support you currently provide to the person you care for and what support you may need to continue your important role.

The assessment is about you as a Carer, it considers the impact of the caring role. It will also consider what difficulties you may be facing, how you manage them and the things that you want to achieve in your day-to-day life.

A Carer assessment provides an opportunity for you to reflect and discuss the impact of your caring role on relevant aspects of your health, wellbeing and social needs, including work, education or training.

What are the benefits of a carer’s assessment?

During a carer’s assessment, the assessor will discuss with you:

  • Information, advice and training
  • Practical support
  • Carer breaks
  • Local community resources and services
  • Financial assistance
  • Emotional and wellbeing support
  • Emergency and future planning
  • Advice about equipment, aids and adaptations
  • Information on carers legal rights and financial support
  • Guidance on services and alternative support such as carer direct payments

Preparing for a carer’s assessment

A carer’s assessment is your opportunity to discuss how caring affects your life and what support you may need. Being prepared can help ensure you get the right help.

You might want to write a day-to-day diary of the caring tasks you do. This can help you understand how much support you provide, what support you already have and what support you may need.

It may also be helpful to think about:

  • How your caring responsibilities influences your daily life such as the practical tasks you do, emotional support you provide and whether caring affects your ability to work, study or socialise
  • Any difficulties you face, such as physical strain, mental health and emotional stress, financial struggles and lack of time for yourself or your own health needs
  • What support would help you

Request a Carer's assessment 

To request assistance from Adult Social Care please complete the below form. You can complete the form for yourself or on behalf of someone you know.

Once submitted, this form will go to the Cheshire West and Chester Community Access Team. You will receive a confirmation email message once the form is submitted and follow up calls if required. The team aim to respond within three working days. Please complete this form to the best of your ability and with as much information as possible.
